The NDIS is responsible for supports that help you go about your daily life and are related to your disability-related impairments that impact your functional capacity. The NDIS fund supports that are related to things you can and can’t do, due to your disability, which you need on an ongoing or regular basis.
This includes maintenance supports, given or supervised by qualified healthcare staff. These are ongoing supports that help you keep your body’s functions and abilities. This could include long term therapy or support, to improve your abilities over time or stop things from getting worse.
The NDIS is responsible for these supports if:

the support is directly related to your disability-related impairments that impact your functional capacity – the things you can and can’t do because of your disability
-
you also need the support so you can continue to live in the community, study or work.

Accessing the NDIS

Once your application is approved you will then work with a Local Area Co-Ordinator (LAC) to assist you in allocating your funding.

NDIS Assessments and Reports

I often hear navigating the NDIS for the first time is like trying to learn a new language, with many of my patients reporting feeling overwhelmed. I'm here to help.
The primary role of NDIS reports from health practitioners is to provide evidence that assists decision-makers at the NDIS. This means it is necessary to provide an official diagnosis of your disability and supporting documentation from a specialist to confirm that diagnosis (eg. multiple sclerosis, postural orthostatic tachycardia syndrome (POTS), anxiety, autism, other). Your application will also need to show that the disability and its impairments are permanent.
At YMPT I have experience with writing NDIS access and review reports and have helped many clients to successfully apply for NDIS support/funding. The report writing process is centered around research so you can be confident the recommendations being made are clear, evidence-based recommendations.
What's The Process?
A well written report starts with getting to know all about you. This means YMPT has a strict intake process before I commence the assessment and reporting writing process. Please note I cannot guarantee acceptance by the NDIS and I reserve the right to decline a report request if I do not think your application is likely to be accepted. If I don't have the capacity or experience to complete an NDIS report, I will refer you to another provider who may be better placed to assist.

Report length 1-3 pages and includes:
A brief review of your diagnosis.
An explanation of your main impairments and their functional impact with general comments about difficulties with mobility, self-care, and community access.
Your treatment to date and your response to that treatment.
Proposed frequency of treatment on the NDIS and a short literature review to support these recommendations.
Other recommendations regarding low cost assistive technology.
Recommendations for other services eg. occupational therapy.
Full Functional Report | $188.42/hr or prorata

Plan Review, Change of Circumstances or Access Request
Report length typically 5-10 pages and includes:
A full review of your diagnosis and history of your condition/s.
Explanation of your diagnosis and its functional impacts.
Past and current treatments.
Detailed review of your capacity within the 6 NDIS domains (mobility, communication, social. interaction, learning, self-care, self-management.
Standardised questionnaires and objective results of assessments with tabulated results.
Evidence based recommendations directly related to your NDIS goals.
Recommendations for NDIS funding into the future.

Then...
1. YMPT will call the patient/carer or guardian/other to make an initial appointment time and will schedule an appointment as soon as practically possible.
2. Depending on the complexity and volume of new patient medical history and other documentation, billable time may be spent prior to the initial assessment, reading, researching and preparing.
3. If applicable, YMPT will provide and seek approval for, a reasonable estimate of time to be spent on the above process.
4. Depending on the complexity of the patient's presentation and circumstances, the initial assessment may need to be conducted over 1-3, typically 60min appointments.
5. If an assessment is required specifically for a report or access letter and recommendation, please make this known prior to the initial assessment to ensure the most appropriate assessment is conducted.
Reports
Please allow 2 weeks from the day of final assessment for a finalised reported to be forwarded. There may be instances where reports are not finalised within this time frame and if this is the case, YMPT will let you know as quickly as possible.

“Where a worker is travelling to provide services to more than one participant in a ‘region’, then it is reasonable for a provider to apportion all of the travel time (including the return journey where applicable) between the participants who received support from the worker.”

Provider Travel
•“For a vehicle owned by the provider or the worker, up to $0.99 a kilometer can be charged.
•“For other forms of transport or associated costs, such as road tolls, parking, public transport fares, up to the full amount”, these will be directly on-charged.
